Pricing

We aim to be fair and transparent.  Monthly membership fee includes all office visits, telemedicine and telephone calls, care coordination, routine labs and most office based procedures. Also included is the annual wellness examination

Adults under age 65 - Individuals $130/month, Families $200/month

Adults age 65 and over - Individuals $170/month, Families $280/month

Children and adults under 26 - Complimentary with Adult membership

Prices may vary based on specific factors, and will be discussed in advance. A registration fee of $200 per family will be added to the first months payment. 

You may cancel membership at any time with no penalty.

Membership is annual, but can be billed monthly upon request.

A limited number of scholarships are available for families in need.
